Our Services

Transformative Nursing provides a comprehensive suite of specialised nursing services designed to support clients with complex health needs in their homes. Our services promote independence, enhance quality of life, and achieve health goals, including:


Complex Bowel Care

We provide safe, evidence-based management for clients with bowel dysfunction, such as those with spinal cord injuries, multiple sclerosis, or neurogenic bowel conditions. Our RNs perform digital rectal examinations, administer enemas, and implement bowel programs (e.g., manual evacuation, suppository use) per clinical guidelines. We focus on dignity, infection prevention (using PPE, sterile techniques), and client education to promote independence, reducing the risk of complications like constipation, impaction, or autonomic dysreflexia.


PEG Feeding and Management

Our expert RNs manage percutaneous endoscopic gastrostomy (PEG) feeding for clients unable to eat orally (e.g., post-stroke, motor neuron disease). We ensure correct tube placement, administer feeds/fluids per dietitian plans, and monitor for complications like blockages, infections, or skin breakdown at the site. We train clients and families on PEG hygiene (e.g., cleaning with saline, rotating the tube) and emergency protocols (e.g., dislodgement), supporting NDIS participants in maintaining nutrition and hydration safely at home.


Urinary Catheter Management

We offer comprehensive catheter care, including insertion (indwelling, suprapubic, or intermittent), maintenance, and monitoring for clients with urinary retention or incontinence (e.g., spinal injuries). Our RNs use aseptic techniques to prevent infections (e.g., CAUTIs), perform bladder scans to assess residuals, and change catheters per schedule (e.g., every 6-8 weeks). Subcutaneous Injections

We follow strict infection control (e.g., alcohol swabs, sharps disposal) and monitor for adverse reactions (e.g., hypoglycaemia). Clients are educated on self-administration where appropriate, empowering NDIS participants to manage their health with confidence.


Complex Wound Management

We provide advanced care for chronic wounds (e.g., pressure injuries, diabetic ulcers) and post-surgical wounds, using evidence-based practices to promote healing. Our RNs assess wounds (e.g., size, depth, exudate), select appropriate dressings (e.g., hydrocolloids, alginates), and manage infections (e.g., debridement). We collaborate with wound specialists, monitor healing progress, and educate clients on pressure relief and nutrition, reducing the risk of deterioration and enhancing quality of life.


Medication Management

We ensure safe administration and monitoring of medications, including oral, topical, and injectable forms, for clients with complex regimens. Our RNs verify prescriptions, use Webster packs for compliance, and monitor for side effects or interactions (e.g., sedation). We educate clients on medication schedules and purpose, ensuring adherence and efficacy while minimising risks like overdose or missed doses, critical for NDIS participants with cognitive challenges.


Ventilator Management and Respiratory Support

Our specialised RNs manage clients requiring mechanical ventilation or respiratory support (e.g., for neuromuscular diseases). We monitor ventilator settings, ensure airway patency, and perform suctioning to prevent blockages. We assess oxygen saturation, respiratory rate, and lung sounds, adjusting care to prevent complications like pneumonia or hypoxia. Clients and families are trained on emergency protocols (e.g., power failure, alarms), supporting safe home care for high-needs NDIS participants.


Tracheostomy and Laryngectomy Care

Our RNs provide active monitoring (e.g., vitals checks, repositioning), administer medications, and respond to emergencies (e.g., seizures, respiratory distress). We ensure a safe sleep environment, document care in real-time (e.g., via Halaxy for NDIS audits), and provide peace of mind for NDIS participants and families.


Monitoring Vitals

We conduct regular checks of vital signs—blood pressure, pulse, temperature, respiratory rate, and blood sugar levels—for clients with chronic conditions (e.g., diabetes). Using medical-grade equipment (e.g., BP monitors, glucometers), our RNs detect abnormalities early (e.g., tachycardia, fever), enabling timely interventions. We document trends in EHR systems and educate clients on self-monitoring, empowering NDIS participants to manage their health proactively.


Health Education

We deliver tailored education for NDIS participants and families, focusing on self-management of conditions like diabetes, wound care, or catheter use.
