Restorative Care Pathway

A change in health, illness, fall or decline in mobility can make everyday life more difficult. With the right support at the right time, many older Australians can regain skills, rebuild confidence and continue doing more for themselves at home.

What is the Restorative Care Pathway?

The Restorative Care Pathway is a short-term pathway available through the Australian Government Support at Home program.

It provides access to intensive nursing and/or allied health services designed to help older people maintain or regain function, improve independence and reduce or prevent further age-related decline.

Restorative care isn't about doing everything for you. It's about working with you so you can continue doing as much as possible for yourself.

An approved restorative care episode can provide support for up to 16 weeks depending on assessed needs and goals.
