Transition Care – Hospital to Home
Leaving hospital is an important milestone, but going home does not always mean recovery is complete. For many older people, the first few weeks after hospitalisation are when additional support matters most.
What is Transition Care?
The Australian Government Transition Care Program provides short-term care for eligible older people following a hospital stay. It is designed to support recovery, optimise independence and help avoid unnecessary long-term care where possible.
Transition Care can generally be provided for up to 12 weeks.
Transformative Nursing & Support Services is a community nursing and support provider. We do not operate or administer the Australian Government Transition Care Program. We work with hospitals, Transition Care providers and other authorised organisations where services are commissioned or referred to us.
