Support Coordinators can refer participants to Transformative Nursing & Support Services for reliable, nurse-led community supports across Townsville and the broader North Queensland region, including Magnetic Island, the Burdekin, and Charters Towers. Our team supports participants with a range of complex needs, including individuals living with complex physical disabilities, those requiring high-intensity nursing supports, and participants transitioning home following a hospital admission. We also work with participants who require behavioural or psychosocial supports and older NDIS participants who may be transitioning toward aged care services such as the Support at Home program. To ensure timely support, all new referrals are acknowledged within 24 hours, with an intake assessment typically arranged within 2–5 days depending on participant needs and service availability.
